A pilot with Meridian Health to reduce 30 day readmission for patients discharged with Congestive Heart Failure (CHF), demonstrated that daily automated interaction supplemented by clinician telephone support on an exception basis reduced 30 day readmissions by 66% versus a control group.   Meridian is currently in the process of implementing this capability system wide while extend it to COPD.   Other chronic diseases are expected to follow.   Click here to get the Meridian Health CHF Pilot Report.

My Internet Lockbox is an online repository and communication vehicle for key records (conditions, medications, etc.), doctors’ information, emergency contacts, forms and directives.   ID cards and engraved jewelry allow emergency responders to access important information instantly.   This information can be retrieved anywhere online and the content can also be spoken, printed, faxed or emailed.   Family members and caregivers can be notified when access to the Lockbox occurs.

Teammate Proximiti understands that for many of our health care service customers, it’s not just the subscriber that can benefit from our services.   Caring for an elderly parent, managing a chronic disease, or wanting to lose weight efficiently is often a family challenge.   Proximiti allows each user or person signing up a family member to designate teammates.   These teammates are notified of interactions with Proximiti, such as a successful check in call, via text message, email or an automated phone call.   It’s a simple way for family members to keep up with new information, Improves coordinated care, and reduces time spent discussing adherence to medically related prescription requirements.   Keeping up with a family member is simple and allows families to coordinate care and to spend time on more pleasant topics than adherence to prescription requirements.  


However, Proximiti’s Health Platform can create any number of custom health services that can be utilized instead of, or in addition to, the above Proximiti products.
