A pilot with Meridian Health to reduce 30 day readmission for patients discharged with Congestive Heart Failure (CHF), demonstrated that daily automated interaction supplemented by clinician telephone support on an exception basis reduced 30 day readmissions by 66% versus a control group.
Meridian is currently in the process of implementing this capability system wide while extend it to COPD.
Other chronic diseases are expected to follow.
