The chronic disease channel currently has support for Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disorder (COPD), Asthma and Hypertension.
We will be adding several others in the next few months.
For each chronic disease, we have prepared a set of daily questions that are designed to provide focus on the habits that are required to control the condition and the symptoms that may indicate the condition is not under control.
Immediately upon conclusion of each session, all answers are assessed and rated on a standard scale:

Should any assessment meet the alarm criteria, then member’s teammates are notified immediately.
In addition, the member and any authorized teammate can see the member’s complete history.
This channel can be very effective when used by itself, but can be even better when used in conjunction with My Assistant and the Lifestyle Channels, such as Lose Weight and Keep It Off or Wellness.
There is a discount available if it is purchase as an add-on to these other products.
